Introduction:
Love & Loyalty Homecare is committed to providing compassionate, high-quality care that allows clients to live safely and comfortably at home. We are expanding our team and looking for a dedicated Registered Nurse (RN) to join us on a PRN (as-needed) basis. Must be able to travel to the following areas Northern Neck, Petersburg, Kilmarnock, Northumberland and surrounding areas.
Position Overview
The Registered Nurse will be responsible for conducting initial home assessments and reassessments for clients, ensuring that care plans meet medical and personal needs. This is a PRN role with flexible scheduling, ideal for nurses seeking supplemental income or a rewarding position outside of traditional clinical settings.
Key Responsibilities:
Lead Clinical Intakes: Manage and process incoming clinical referrals for both skilled care and personal care clients.
Conduct Assessments: Perform comprehensive, accurate initial clinical intake assessments for incoming home health patients.
Evaluate Criteria: Review medical records, hospital discharge paperwork, and physician orders to verify skilled care necessity.
Collaborate with Leadership: Complete initial 1-on-1 direct training alongside our Chief Nursing Officer to master internal clinical processes.
Coordinate Care Pipeline: Partner seamlessly with hospital discharge planners, physicians, and families to safely transition patients to our services.
Document Accurately: Maintain meticulous digital clinical documentation complying with state regulations and agency standards.
Qualifications & Skills:
Licensure: Active, unencumbered RN or LPN license in the state of Insert State .
Experience: Minimum 1 2 years of clinical nursing experience; home health, private duty, or skilled nursing background is highly preferred.
Assessment Skills: Proven hands-on clinical assessment experience with a sharp eye for detailed patient changes and medical necessity.
Reliability: Strong organizational skills and the ability to dedicate focus to a rapidly scaling business.
Tech-Savvy: Comfortable utilizing Electronic Medical Records (EMR) software and digital communication platforms.
Communication: Excellent interpersonal skills to work effectively with patients, families, providers, and our CNO.
