Overview:
RN/LPN Part Time - 10 hours per week
8:30-1:30 2 days per week
RiverWoods is currently seeking a RN or LPN staff member to provide direct resident care in a team-focused environment where our residents are at the heart of what we do. This role would be responsible for supporting our Wellness Clinic in Independent Living under the direction of the Wellness nurse practitioner. The ideal candidate would be available to work 2 days a week.

Responsibilities:
- Performs professional duties in accordance with facility policies/procedures and standards of professional practice, State of New Hampshire Nurse Practice Act, OBRA and HIPAA.
- Collaborates with interdisciplinary team members to provide patient-centered care for independent residents. Participates in interdisciplinary team meetings.
- Assesses and documents changes in residents’ physical or mental status and reports findings to the primary care provider or Wellness Clinic nurse practitioner. Provides complete clinical data to facilitate decision-making.
- Maintains standing visits with residents for routine health monitoring, wound care, and assistance with medication management.
- During resident walk-in clinic hours, independently triages and treats minor illness and injury as appropriate within nursing scope, elevating to a higher level of care when necessary. Responds to urgent resident needs via “R-call” alert system; initiates and supports EMS response as appropriate.
- Provides leadership and direction for direct caregivers. Supervises direct care through observation and evaluation of direct care results.
- Obtains, administers and documents medications and treatments given to residents following professional guidelines and department policies. Inventories and maintains locked system for narcotics.
- Establishes positive working relationships with team members, residents and family members in order to facilitate open, direct communication.
- Supports, creates, and maintains a positive culture for team members and residents.
Qualifications:
- Must possess, as a minimum, a Nursing Degree or Diploma from an accredited college, University or nursing school.
- Long term care facility and specialized knowledge of nursing care of the elderly and rehabilitative nursing is preferred.
- Must hold a registered nursing (RN, LPN) license within the State of New Hampshire. Certified in intravenous administration preferred. Must maintain CEU's annually. CPR is required.
