Job Overview

We are seeking an experienced Social Worker to provide direct services to pediatric and adolescent populations in a home care / school-based setting. This role involves working closely with students, families, and school staff to address social, emotional, and environmental challenges.

Key Responsibilities

• Provide direct social work services using the five phases of the social work process
• Collaborate with students, families, and school staff to develop effective care plans
• Coordinate care to ensure quality outcomes and efficient use of resources
• Perform case management and discharge planning
• Refer patients to community resources and agencies
• Support pediatric/adolescent populations with social and lifestyle challenges
• Maintain clear communication with all stakeholders

Required Qualifications

• Master's Degree in Social Work (MSW)
• Active LMSW or LLMSW License - Michigan
• 5+ years Case Management experience
• 2+ years recent Acute Inpatient Case Management (MANDATORY)
• Discharge Planning experience
• Valid Michigan Driver's License & insured vehicle
