Job Summary
Springbrook Center in Westbrook, Maine, is seeking a Physical Therapist (PT) for a permanent, full-time position in a skilled nursing facility.
Springbrook Center provides post-hospital care, short-term rehabilitation, long-term care, and respite services. The facility has earned the American Heart Association's Skilled Nursing Facility Heart Failure Certification.
The Physical Therapist will support residents' mobility, functional independence, and rehabilitation goals through individualized therapy services.
Key Responsibilities
Evaluate residents' physical function, mobility, and rehabilitation needs.
Develop and implement individualized treatment plans.
Provide therapeutic interventions to improve strength, balance, coordination, and mobility.
Monitor and document resident progress toward established goals.
Collaborate with nursing staff and other healthcare professionals.
Educate residents and caregivers on therapeutic exercises, mobility, and safe movement.
Maintain accurate clinical documentation and follow applicable care standards.
Qualifications
