  • New office location in Lady Lake, FL with an existing patient panel
  • Lower daily patient volume focused on comprehensive patient care
  • Outpatient-only schedule with no nights, weekends, holidays, or hospital responsibilities
  • Monday"Friday schedule
  • Provide care for the senior patient populations
  • Focus on prevention, quality outcomes, and evidence-based medicine
  • Manage acute and chronic conditions including diabetes, COPD, CHF, obesity, and cardiovascular disease
  • Team-based support including care coordination, pharmacy resources, and data tools
  • Opportunity to supervise Nurse Practitioners and Physician Assistants
  • Established patient base in a managed care setting
  • EMR-based practice environment
  • Seeking physicians with strong diagnostic and chronic disease management experience
  • Active Florida medical license
  • Minimum 3 years of outpatient primary care experience preferred
