AMERIPRIME HOME HEALTHCARE INC. looking to hire a Full Time Physical Therapists (PT) in the Central Valley area.

Compensation: Salary non exempt - Depending on experience

• Annual Salary range $135,200.00-$141,440

Areas of Responsibility:

• Makes initial admission/evaluation visits
• In consultation with the assigned Clinical Team Manager initiates and regularly re-evaluates and revises the plan of care. Instructs assigned PTA on the plan of care revisions
• Provides accurate and timely documentation consistent with the plan of care
• Communicates with the Clinical Team Manager or with the physician and other members of the home care team informing them of significant changes in the patients' condition and needs. Documents these communications
• Follows established standards for point of service technology, documentation, and synchronization
• Submits all documentation according to agency requirements. Meets established productivity standards
• Performs other duties as assigned consistent with skills and training

Qualifications:

• Excellent assessment skills and excellent interpersonal skills
• Positive attitude and ability to confront problems constructively
• highly motivated, self-directed, flexible, and able to work well and participate in a multidisciplinary team setting

Requirements:

• Graduate from an accredited physical therapy program approved by the Council on Medical Education of the American Medical Association in collaboration with the American Physical Therapy Association
• Current California PT License
• Minimum one year home health experience
• Valid California driver's license and automobile insurance
