Health Home Care Manager

Job Title: RN - Home Health/Hospice

Location: Bryant, AR, 72022

Contract: 1.5 Months possible contract extension

Shift: Mon to Fri - 08:00 AM to 04:30 PM

Weekly Hour Guarantee: 32

Coverage Areas: Pulaski, Saline, Jefferson and surrounding counties.

What are the top responsibilities for this position? (Please be as detailed as possible): Care of the hospice patient in their home setting. Participation in IDG meetings, case management of patients. Will include daily travel between patient's places of residence.

Must have qualifications/experience: RN with HCHB knowledge and experience. Great communication skills and bedside manners

What skills/attributes are nice to have, and will set a candidate apart? Performing a hospice needs assessment and admission

Type of certification required: Minimum of Associate Degree in Nursing

On Call/Weekend Requirement: Negotiable

Will Mileage be reimbursed between visits, from first patient to last patient (Y/N)? Yes
