  • Approximately 15 patients per day
  • Monday-Friday, No nights or Weekends

  • Integrated team including NP/PAs, MAs, social workers, lab/pharmacy support, and referral coordination Qualifications
    • Board Certified or Board Eligible in Family Medicine or Internal Medicine
    • Geriatricexperience preferred
