Registered Nurse (RN) – Home Care




Location: Nassau County (with occasional cases in Queens, Manhattan, and Brooklyn, NY)




Schedule: 20-30 hours per week | Flexible Scheduling | PTO

Position Summary


We are seeking an outstanding Per Diem Registered Nurse (RN) to join our growing team. This role offers flexibility, a very competitive compensation, and an opportunity to work in a supportive, professional environment. Most assignments are in Nassau County and Queens, with some casses in Manhattan, and Brooklyn.


The RN will provide clinical supervision of home health aides, conduct patient in-home assessments, develop care plans, and collaborate with our team to ensure continuity of care.

Key Responsibilities



  • Conduct in-home patient assessments and develop individualized care plans.

  • Supervise, support, and train home health aides to maintain quality of care.

  • Communicate effectively with patients, families, and interdisciplinary team members.

  • Document clinical findings and maintain compliance with agency and state regulations.

  • Work both independently and collaboratively within a team setting.

  • Availability to work some weekends as needed.




Qualifications



  • Active New York State RN license (required)

  • Strong communication and interpersonal skills

  • Ability to work both independently and as part of a team

  • Excellent organizational and clinical assessment skills

  • Reliable transportation to travel to patient homes in Nassau County, Queens, Manhattan, and Brooklyn
