Sagara Health is building AI specifically for care management. Its platform, NexCM, captures care-management conversations, transcribes them, identifies the information that matters, and turns those conversations into structured documentation and follow-up actions.
The goal is simple: reduce the administrative burden on care managers so they can spend more time supporting patients and members.
Sagara was founded by an experienced managed-care leadership team with more than 70 years of combined experience across Medicaid, Medicare Advantage, health-plan operations, value-based care, healthcare technology, and care management.
The team has previously built and scaled health plans, provider networks, healthcare operations, and technology programs across major U.S. healthcare organisations.
Sagara is now entering an important commercial stage, with initial pilot activity beginning and a broader strategy to expand across health plans, ACOs, at-risk providers, and other care-management organisations.
As the company enters this next phase, Sagara is selectively expanding its Board of Advisors with senior healthcare executives, founders, operators, and investors who can contribute strategic perspective, industry relationships, and long-term alignment.

NexCM is designed specifically around that workflow. The platform supports the journey from: Care-management conversation → transcription → structured information extraction → care-manager review → structured documentation → follow-up actions → existing care-management systems.
Rather than applying general-purpose AI to healthcare, Sagara is focused on a specific workflow the founding team understands deeply.
