Key Responsibilities

  • Perform comprehensive clinical assessments to understand the psychological, social, and emotional needs of hospice patients.

  • Develop and implement tailored treatment plans utilizing evidence-based intervention techniques.

  • Facilitate therapeutic interventions such as individual and family counseling, psychoeducation, and support groups.

  • Document all clinical interactions, assessments, and treatment plans accurately and in a timely fashion.

  • Work collaboratively with physicians, nurses, and other healthcare staff to provide coordinated care to our patients.

  • Offer emotional support and resources to patients’ families, ensuring they receive the guidance necessary during challenging times.

  • Participate in regular team meetings to discuss patient care strategies and stay updated on best practices in hospice care.

Patient Population / Client Focus

In this role, you will engage with a diverse clientele comprised of terminally ill patients and their families. You will be instrumental in addressing the unique emotional and social challenges presented at the end of life, focusing on dignity, compassion, and quality of life improvements. Your work will directly impact patients' peace of mind and well-being as they navigate their final chapters.

Qualifications

  • Master’s degree in Social Work (MSW) from an accredited program.

  • Active Licensed Clinical Social Worker (LCSW) license in the state of Tennessee.

  • Comprehensive knowledge of hospice care principles and an understanding of the psychosocial dynamics of terminal illness.

  • Exceptional communication and interpersonal skills to effectively engage with difficult and delicate conversations.

Preferred Experience

  • Experience in palliative care, hospice services, or other relevant settings is highly preferred.

  • Proficiency with electronic health records (EHR) systems to track patient progress and treatment plans.

  • Background in solution-focused therapy techniques and an understanding of grief counseling methodologies.

Compensation and Benefits
