Make a meaningful impact on the lives of Veteran residents in a long-term care setting.</p><h3>Responsibilities</h3><ul> <li>Provide practical nursing care and treatment for Veteran residents with medical, physical, cognitive, and behavioral health needs.</li> <li>Implement individualized nursing care plans and perform direct resident care in coordination with the interdisciplinary team.</li> <li>Prepare, administer, and accurately document medications and treatments prescribed by physicians.</li> <li>Observe, assess, and report changes in residents’ physical, cognitive, and emotional conditions.</li> <li>Take and record vital signs, collect specimens, and maintain accurate clinical documentation and progress notes.</li> <li>Participate in Veteran-centered rehabilitation, restorative, and habilitation programs, including assistance with activities of daily living and therapeutic activities.</li> <li>May serve in a charge role for a unit or care area, supervising unlicensed personnel and supporting continuity of care.</li> <li>Perform nursing duties in accordance with federal/state regulations, facility policies, Veterans’ care standards, and LPN scope of practice.</li> <li>Collaborate with RNs, physicians, and interdisciplinary team members to support resident care planning and outcomes.</li> <li>Respond to changes in resident condition and escalate clinical concerns to appropriate licensed personnel.</li> <li>Maintain compliance with infection control, safety protocols, and quality care standards.</li> <li>Participate in care planning meetings, interdisciplinary reviews, and ongoing resident evaluations.</li></ul><h3>Requirements</h3><ul> <li>Current Licensed Practical Nurse (LPN) license issued by the Pennsylvania State Board of Nursing, or a valid non-renewable temporary practice permit.</li> <li>2 years of experience preferred.</li> <li>AHA BLS certification.</li></ul><br />#ITLTCPandologic.
