Geriatrician - Associate Medical Director - San Diego, California

San Diego, CA | Full-Time | On-Site | 70% Clinical / 30% Leadership

A growing, mission-driven PACE program in San Diego is seeking an Associate Medical Director to provide site-level clinical and administrative leadership at centers in Downtown San Diego.

This is an outstanding opportunity for an experienced Family Medicine or Internal Medicine physician leader who wants to remain clinically engaged while expanding their impact through provider mentorship, interdisciplinary leadership, quality improvement, utilization management, and population health.

• 70% clinical / 30% administrative leadership
• Only 3-5 patient encounters per clinical day
• Protected administrative time for leadership and provider development
• Direct mentorship of physicians, NPs, and PAs
• Lead interdisciplinary team (IDT) collaboration

The Associate Medical Director will partner directly with the Medical Director to provide clinical leadership at the assigned PACE center. The role combines direct patient care with meaningful physician leadership, allowing the successful candidate to influence both individual patient outcomes and the broader quality of care delivered across the center.

• Provide comprehensive primary care to medically complex and geriatric patients
• Conduct new participant evaluations
• Manage acute and chronic conditions
• Provide clinical oversight for participants in SNF and ALF settings
• Participate in interdisciplinary care planning

• Provide clinical mentorship to physicians and APPs
• Serve as a resource for complex clinical decision-making
• Support provider development and clinical remediation
• Facilitate clinical education and didactic sessions
• Support site operations and workflow improvement

Required:

• MD or DO
• Board Certified in Family Medicine or Internal Medicine
• 3-5+ years of post-residency clinical experience
• Experience caring for geriatric or medically complex populations
• 2-3+ years of clinical leadership, medical directorship, administrative leadership, or GME faculty experience
• California medical licensure or eligibility

Preferred:

• Geriatrics fellowship or board certification
• Hospice & Palliative Medicine fellowship or board certification
• PACE experience
• Experience leading interdisciplinary teams
• Population health/value-based care experience
• Experience mentoring physicians and APPs
