Job Description:

The LPN / LVN participates, coordinates and provides direct nursing care in a manner which ensures participant's satisfaction and retention. Coordinating with the physician, RN, transportation and home care. Assists physicians and nursing staff with a variety of tasks, including but not limited to vital signs, wound care, medication administration/management, treatments, patient teaching, venipuncture, catheter care foot care, and participant set up for physician visit.

Required Qualifications:

  • Education to meet certification, license or registration requirement.
  • Upon hire: Washington Licensed Practical Nurse
  • Upon hire: CPR and First Aid Certification (American Heart Association)
  • 1 year Long term care or related geriatric experience.
  • Experience working with the frail and elderly.
