Work EnvironmentThis position is primarily remote, allowing for flexible work hours and the ability to tailor your schedule around client needs while ensuring continuous care. Your work would typically take place from Monday to Friday, 8:00 AM to 4:30 PM, with occasional evening or weekend hours based on client availability. Key Responsibilities- Conduct comprehensive assessments of seniors' psychosocial needs, focusing on emotional, mental, and social factors to craft tailored care plans.
- Facilitate personalized care discussions with clients and families, addressing goals, preferences, and any adjustments needed along the way.
- Provide individual and family counseling to assist in navigating life transitions, addressing challenges, and reinforcing emotional resilience.
- Serve as an advocate for clients by ensuring their rights are protected and that they have access to necessary community resources and services.
- Collaborate with interdisciplinary teams, including healthcare professionals and caregivers, to coordinate comprehensive care and enhance client well-being.
- Perform regular check-ins with clients through virtual meetings or phone calls to monitor their situations and living conditions, ensuring a safe environment.
- Manage conflicts and facilitate effective communication within families involved in a senior client's care, helping to foster understanding and cooperation.
- Educate clients and their families about available resources, support groups, and community programs to promote engagement and enhance quality of life.
- Maintain meticulous documentation of assessments, care plans, progress notes, and client interactions in compliance with ethical standards.

Qualifications- A Master’s degree in Social Work from an accredited institution is required.
