WE CAN ONLY LOOK AT PROVIDERS WITH CLEAN FILES If you present at provider with malpractice or an NPDB that is not clear, we will not name clear the candidate and decline. What we need:
- A "clean" applicant
o Physicians with large settlements, DUIs/drug issues, restrictions, terminations, Medical Board actions, etc. are not eligible for temporary privileges
- The RFA, CV, and 24 months of relevant activity
- A completed application separate from / following the above step
- References on standby to respond immediately

- Shift Times: 7am-4pm
- Lunch 1 hour
- Provider Min # Shifts Required: see above
- Guaranteed Hours per Day: 8
- Hours Worked for OT to apply: 8 per day
- Rates to be Filled Out on Bid: Clinic only: Regular, Orientation, Overtime, Holiday

- Current CV (must be uploaded to bid)
- License: Active only
- DEA Required: Yes
- Boards: BC only
- Skills/Certs: BLS, ACLS, PALS
Please note the following travel deviations from our contract will apply to the assignment (or current confirmation agreement):
- NOT reimbursed:
o Mileage from provider's home to departing airport, as well as from airport to home upon return.
o Ride share (Lyft/Uber/etc.) and/or taxi from provider's home to departing airport, as well as from airport to home upon return.
- WILL be reimbursed:
o Airport parking at provider's departing airport if provider drives themselves to the airport.
o Insurance policies for rental cars must be pre-approved, prior to booking, otherwise it will not be reimbursed.
o Ride share (Lyft/Uber/etc.) and/or taxi from destination airport to hotel/lodging for assignment, as well as from lodging to hospital/clinic, and return tr
