Home Health Physical Therapist (PT)

Located in the Sacramento region

As a Home Health Physical Therapist, your key responsibilities will include:

  • Delivering home-based physical therapy to patients who have experienced a decline in mobility, strength, and functionality due to illness or injury
  • Collaborating with a home health team that may consist of an RN, LPN, CNA, HHA, OT, and SLP, focusing on the patient’s care plan and objectives
  • Monitoring movements, performing assessments, documenting progress, and adjusting care plans as necessary
  • Guiding patients through exercises and hands-on therapy to manage pain and enhance mobility
  • Informing patients about their care plans, established goals, exercises, and self-care techniques
  • Coaching family members on the care plan, therapy objectives, and safe exercise assistance

Essential qualifications for Home Health Physical Therapists include:

  • Graduation from an accredited Physical Therapist Program and possession of an active PT license in California
  • At least 1 year of experience in physical therapy, preferably in home healthcare settings
  • Current CPR certification
  • Familiarity with state and federal regulations governing home health care
  • Strong clinical judgment, excellent interpersonal skills, dexterity, and compassion
