Job Profile Summary

This position is centered on delivering both professional and non-professional nursing care services as per physician directives. The role encompasses various Clinical Nurse responsibilities, including assessing, planning, implementing, and evaluating the care requirements of a specific group of patients. It also involves providing nursing care and executing identified clinical support tasks within the defined scope of practice to fulfill the patient’s care plan as instructed. The role demands a valid nursing license and is characterized as a professional individual contributor position that may oversee the work of junior professionals or manage specific processes and programs. A significant portion of the role involves supervising the design, implementation, or delivery of processes, programs, and policies, utilizing specialized knowledge and skills typically gained through advanced education. This senior-level position necessitates extensive knowledge of the job area, usually acquired through advanced education and relevant work experience. Responsibilities typically include managing projects and processes, working autonomously with minimal supervision, mentoring and reviewing the work of junior professionals, and resolving complex issues.

Job Overview

The individual in this role will be responsible for providing diabetes self-management education to patients and their families as they prepare for discharge. Patients with complex health conditions and specific learning needs will be referred for health education and counseling to promote wellness and prevent both acute and long-term complications.

Job DescriptionMinimum Qualifications:
  • Registered Nurse (RN) Licensure in Massachusetts.
  • A minimum of three (3) years of RN experience.
  • Current Basic Life Support (BLS) Certification.
  • Certification in Diabetes Education (CDE) or the requirement to obtain it within one year of hire.
Preferred Qualifications:
  • Bachelor of Science in Nursing (BSN).
Duties and Responsibilities:

The following duties and responsibilities are intended to outline the general nature of the work and are not exhaustive. Additional tasks may be assigned as necessary.

  • Facilitates patient self-management initiatives within a family-centered program, employing a multidisciplinary approach to manage patients with various types of diabetes (Type 1, Type 2, steroid-induced, and cystic fibrosis-related) and their families.
  • Collaborates with patients diagnosed with diabetes and their families to enhance self-management goals tailored to their age, providing lifestyle coaching and conducting individual patient visits in a hospital clinic setting, as well as group sessions.
  • Delivers patient education while collaborating with external community resources.
  • Manages cases, including oversight and interpretation of laboratory results for diabetic patients, ordering therapeutics, durable medical supplies, and prescription refills, as well as coordinating referrals and diagnostic studies as necessary.
  • Acts as an expert clinician and diabetes educator to enhance patient care and clinical outcomes within the hospital environment.
  • Engages in and maintains awareness of performance improvement initiatives.
  • Evaluates and integrates evidence-based practices and research into clinical work.
  • Conducts and accurately documents clinical assessments.
  • Develops, implements, and assesses individualized care plans for patients.
  • Utilizes professional nursing judgment to tailor care plans based on patient assessments.
  • Manages patient assignments by delegating tasks effectively based on patient needs and team members' skills.
  • Creates a comprehensive educational plan for patients and families, utilizing appropriate resources and documenting according to departmental standards.
  • Respects patient and family rights by supporting informed decisions and advanced directives.
  • Continuously evaluates and updates care plans in response to changes in patient health status.
  • Develops a thorough discharge plan using suitable resources and referrals, including community resources.
  • Assists in the onboarding of new staff members and facilitates their integration into the unit.
  • Mentors department staff to foster their professional development through precepting.
  • Engages in professional growth by identifying learning needs and pursuing relevant education.
  • Participates in departmental committees as required.
Physical Requirements:
  • Occasional lifting of 30-35 lbs and assisting with patient mobility.
  • Prolonged periods of standing and walking.
  • Occasional twisting, bending, reaching, pushing/pulling, sitting, kneeling, and squatting.
  • Exposure to infectious diseases and potential carcinogenic substances.
  • Risk of exposure to blood-borne pathogens.
  • Requires fine manual dexterity for patient handling.
  • Requires visual and auditory acuity to monitor patient care effectively.

  • Strong communication skills to interact effectively with patients, families, physicians, and the healthcare team.
  • Knowledge and application of the nursing process.
