This is an exciting opportunity for a clinically driven pharmacist who is passionate about population health, chronic disease management, and improving patient outcomes through collaborative care.

In this role, you will:

• Work directly with members managing chronic conditions, including diabetes, COPD, CHF, asthma, and metabolic syndrome

• Utilize predictive analytics and clinical data to identify and support high-risk populations

• Collaborate with physicians and interdisciplinary teams to optimize medication therapy and care plans

• Conduct wellness visits, medication reviews, lab monitoring, and patient education

• Support evidence-based interventions designed to improve quality outcomes and reduce unnecessary healthcare utilization

Qualified candidates will possess:

• PharmD or Pharmacy degree from an accredited institution

• Indiana Pharmacist licensure or eligibility

• Strong clinical and patient engagement skills

• Ambulatory care or clinical pharmacy experience preferred

• PGY1/PGY2 residency and BCACP or BCPS certification preferred
