Social Worker

Now Hiring: Social Worker (SW) – Bronx, NY

DUTIES:

• Maintain and/or improve residents’ ability to manage everyday physical, mental, and psychosocial needs

• Identify individual social and emotional needs

• Develop and maintain individualized care plans

• Participate in planning for admissions, community re-entry, or transfers to other facilities

• Collaborate in interdisciplinary care plan meetings to address residents’ social and emotional needs

REQUIREMENTS:

• Bachelor of Social Work (BSW) or Master of Social Work (MSW) required

• Degree must be in Social Work (candidates with Human Services or Psychology degrees will not be considered)

• Long-term care or subacute experience required
