Job Title: Travel Occupational Therapist (OT) - Home Health
Coverage Area: Harford County and Baltimore County, Maryland
Shift Schedule: Monday-Friday, 8:00 AM - 4:30 PM
Mileage Reimbursement: Yes (between patient visits, first patient to last patient)
Job Summary
We are seeking a dedicated and patient-focused Travel Occupational Therapist (OT) to provide high-quality home health services across Harford and Baltimore Counties. This role focuses on helping patients improve independence, safety, and functional performance within their home environment. The ideal candidate is clinically skilled, collaborative, and passionate about improving patient outcomes from start of care through discharge.
Key Responsibilities
Perform comprehensive occupational therapy evaluations and ongoing patient assessments in the home setting.
Develop, implement, and update individualized plans of care designed to achieve patient-specific occupational therapy goals.
Provide skilled occupational therapy interventions to improve independence with activities of daily living (ADLs) and instrumental activities of daily living (IADLs).
Instruct patients and caregivers on home exercise programs, energy conservation techniques, adaptive equipment use, and home safety strategies.
Coordinate care with physicians, nurses, therapists, and the interdisciplinary care team to ensure continuity and quality of care.
Monitor patient progress, document outcomes, and modify treatment plans as appropriate.
Focus on preventing hospitalization, reducing fall risk, and improving overall patient outcomes throughout the episode of care.
Complete accurate and timely clinical documentation in accordance with regulatory, payer, and organizational requirements.
Maintain compliance with all federal, state, and agency policies and quality standards.
Required Qualifications
Unencumbered Occupational Therapist (OT) license in the state where you reside.
Eligibility and authorization to practice Occupational Therapy in the state of Maryland.
Current CPR certification.
Valid driver's license with reliable transportation.
Ability to work independently in a field-based home health environment.
Preferred Qualifications
OASIS certification or experience.
Prior home health or travel therapy experience.
Strong clinical documentation and assessment skills.

Work Environment & Travel Requirements
