We are seeking an organized, empathetic, and motivated Care Coordinator to join our growing team. This is a meaningful opportunity to make a direct impact on the lives of vulnerable adults while working in a supportive and mission-driven environment.
About the Role
As a Care Coordinator, you will serve as a trusted advocate and central point of connection for clients, families, providers, and community resources. You will spend approximately half your week conducting in-person client or care team meetings and the remainder coordinating services, documentation, and follow-up.
This position supports individuals who are chronically ill or living with neurological or mental health diagnoses. Many clients reside in long-term care settings. A full-time caseload ranges from 25-50 clients
Your work will help ensure each client's safety, dignity, and greatest degree of independence within the least restrictive environment
Key Responsibilities
- Promote timely access to appropriate medical and supportive care
- Develop, implement, and monitor individualized care plans
- Coordinate communication between clients, families, hospitals, clinics, specialty providers, and community partners
- Manage transitions of care and referrals
- Support personal care needs (groceries, personal care items, durable medical goods)
- Maintain complete and accurate electronic records
- Identify at-risk clients and contribute to program improvement efforts
- Reassess client status and adjust care plans as needs evolve
Qualifications
- 3-5 years of experience in clinical or community-based care settings (reduction considered for licensed professionals)
- Strong organizational skills and ability to maintain accurate documentation
- Comfort using communication technologies and electronic systems
- Knowledge of local healthcare and community resources is a plus
- Valid driver's license and reliable personal vehicle required
Schedule & Work Environment
- Full-time: Monday-Friday, 8:00 a.m. - 5:00 p.m. (Part-time may be considered)
- Travel throughout Vermont and New Hampshire required
- Remote work available when not traveling
- Company cell phone provided
