Location: Territory expansion into the Concord, NH area (Boscawen, Canterbury, Loudon, Bow, Concord, down to Manchester)
Position Type: Full-Time, Salaried position, 8a-4p, M-F + On-call requirements
About the Company
We are partnering with a Home Health & Hospice organization who is in need of a Director of Clinical Operations due to their continued growth. They are seeking a dynamic and experienced Clinical Director to join their leadership team to lead both their Home Health and Hospice clinical services in a new territory. This is a high-impact leadership role responsible for building, guiding, and elevating their clinical programs, ensuring every patient receives compassionate, high-quality, and compliant care.
About the Role
The Clinical Director provides strategic and operational leadership for all Home Health and Hospice clinical services. This individual oversees clinical staff, patient care delivery, regulatory compliance, and quality outcomes across the organization. As a key member of the leadership team, you will help shape the future of our agency, building strong clinical programs, developing teams and driving excellence in patient care.
Responsibilities
Clinical Leadership & Oversight:
- Lead and oversee all Home Health and Hospice clinical staff and services
- Direct patient care operations, including admissions, case assignments, and care coordination
- Ensure patient needs are continually assessed and individualized plans of care are developed, implemented, and updated
- Provide mentorship, coaching, and performance management to clinical team members
- Facilitate interdisciplinary team collaboration and case conferences to deliver individualized patient care
Clinical Operations & Compliance:
- Ensure delivery of safe, high-quality care in compliance with all federal, state, and local regulations
- Develop, implement, and oversee clinical policies and procedures
- Monitor documentation, clinical outcomes, and key performance indicators
- Work with QA & HR to design, implement & evaluate orientation, training & evaluation of clinical staff.
Strategic Leadership & Growth:
- Partner with executive leadership to develop and achieve organizational goals
- Support program development and service line growth for both Home Health and Hospice
- Build and maintain strong relationships within the healthcare community to promote referrals
- Foster a culture of accountability, collaboration, and mission-driven care
Qualifications
- Active Registered Nurse (RN) license in New Hampshire, in good standing
- Minimum 3-5 years of Home Health and/or Hospice experience
- Minimum 3 years in a clinical leadership role (Director, Manager, Supervisor)
- Strong knowledge of Home Health & Hospice regulations, including CHAP standards
- Experience with OASIS and hospice documentation requirements
- Demonstrated ability to lead teams, manage operations, and drive outcomes
- Excellent communication, leadership, and organizational skills
- Reliable transportation, valid driver's license, and current CPR certification
