American Traveler is seeking a Home Health Physical Therapist with at least 1 year of recent experience, OASIS and HCHB proficiency, and flexibility to cover a wide service territory in Alpena, MI.
Details- Home health setting serving a geriatric patient population
- Caseload of approximately 5-7 patients per day (to be confirmed during interview)
- Service territory covers up to a 60-mile radius from the hub/office
- Day shift schedule, 5x8-hour shifts (8:00 AM - 5:00 PM)
- On-call required every 3rd to 4th weekend
- EMR: HCHB (experience required)
- iPad provided for documentation in the field
- Minimum 1 year of recent home health PT experience required
- OASIS experience required
- HCHB EMR experience required
- Current BLS certification required
- Active PT license required if applicable
- Must be flexible with service territory coverage
- First-time travelers are welcome to apply
- Local candidates are accepted and eligible for the travel rate
- Timely documentation and prompt communication are expected
- Scrubs required in any color; jeans are not permitted
- Mileage is reimbursed at the IRS standard rate
- Personal cell phone is not provided; an iPad is supplied for field use
- All time-off requests must be submitted at the time of initial consideration
- 2 professional supervisory references from within the past 2 years required for consideration
- Full 7-year work history with all gaps explained required for consideration
