Matrix Medical Network is seeking a compassionate and adaptable Nurse Practitioner - PRN to join our Healthcare & Medical Services team. In this flexible, as-needed role, you will deliver high-quality primary and preventive care through a combination of in-home visits and virtual encounters for diverse adult and senior populations. Drawing on your strong clinical background, you will perform comprehensive health assessments, manage chronic and acute conditions within your scope of practice, and support quality improvement initiatives that help payers and government agencies improve outcomes and reduce costs. As a Nurse Practitioner with Matrix Medical Network, you will conduct detailed histories and physical examinations, complete health risk assessments, order and interpret appropriate diagnostic tests, and develop evidence-based care plans. You will prescribe and manage medications where authorized, provide patient and family education on disease management and preventive care, and coordinate with primary care providers, specialists, and care managers to ensure continuity of care. You will accurately and thoroughly document all encounters in the electronic health record (EHR), adhering to clinical, regulatory, and documentation standards. You will collaborate closely with a multidisciplinary team and contribute to quality assurance and population health efforts that support our health plan and government partners. Success in this role requires strong clinical judgment, excellent communication skills, and the ability to work independently in varied community and home settings while maintaining a patient-centered focus. As a PRN provider, you will have the flexibility to accept assignments based on your availability while still playing a critical role in the delivery of innovative, value-based healthcare.
