Clinical Pharmacist

Pride Health is hiring a remote contracted clinical pharmacist in Louisiana! The Clinical Pharmacist will report to the Operations Team Manager. This role is primarily responsible for performing telephonic consultations with members to complete comprehensive medication reviews, adherence adherence-related consults, and improve disease state management. In addition, the clinical pharmacist will document the care delivered and communicate essential information to the member's primary care providers.

Pay Rate: $3,120/week

Duration: 13 Weeks

Shift: 8-hour shifts (varied start times)

Schedule Options:

• 6:00 AM - 2:30 PM
• 7:00 AM - 3:30 PM
• 8:00 AM - 4:30 PM
• 11:00 AM - 7:00 PM
• 3:00 PM - 11:00 PM

Position Overview

We are seeking a skilled Pharmacist to provide high-quality pharmaceutical care in a hospital setting. The role involves medication dispensing, clinical consultation, and ensuring safe and effective drug therapy for patients.

Key Responsibilities

• Review, verify, and dispense medications per physician orders
• Monitor patient drug therapies for safety, interactions, and effectiveness
• Maintain accurate patient medication records in EMR systems
• Provide clinical consultations and drug information to healthcare staff
• Educate patients and families on medication use and therapy plans
• Supervise pharmacy technicians and support staff
• Participate in antimicrobial stewardship and quality improvement initiatives

Clinical Duties

• Evaluate medication appropriateness, dosing, and contraindications
• Support pharmacokinetics and renal dosing protocols
• Identify and report adverse drug reactions
• Assist with medication reconciliation and transitions of care
• Participate in interdisciplinary rounds

Requirements

Doctor of Pharmacy (PharmD) or BS Pharmacy degree

Active State Pharmacist License

BLS or ACLS certification required

PALS required (for pediatric practice)

Hospital experience preferred
