Pride Health is currently seeking candidates for Medical Social Worker will provide support for our client's healthcare systems in Oklahoma City, Oklahoma, this is a contract assignmentwith a high likelihood of extension and a great way to start working with a top-tier healthcare organization!

Job Description:

Job Title:Medical Social Worker

Location: Oklahoma City, Oklahoma

Schedule: 40 hours/week

Shift: Day: Friday, Saturday, Sunday + every other Thursday

Contract Length: 13 Weeks

Pay Range for Traveler: $1,750.00- $1,820.00 Weekly DOE

Job Summary:

• Provide social services to patients and families within a hospital setting
• Support discharge planning, crisis intervention, and psychosocial care

Key Responsibilities:

• Provide social work services to assigned patients and families
• Conduct discharge planning evaluations and gather necessary information
• Perform psychosocial assessments and communicate findings to care team
• Counsel patients and families on medical, emotional, social, and financial concerns
• Provide crisis intervention (e.g., abuse, neglect, high-risk situations)
• Act as liaison between patients, families, hospital staff, and community agencies
• Assist with discharge planning and referrals for ongoing care
• Coordinate post-hospital placements or transfers to post-acute facilities
• Support patients and families in coping with illness and life changes
• Advocate for patients regarding care decisions, ethics, and end-of-life planning
• Evaluate effectiveness of services and adjust care plans as needed
• Maintain documentation of services and interventions
• May supervise students or new staff
• Perform additional duties as assigned

Required Skills & Experience:

• Minimum 2 years of Social worker experience .
• Knowledge of social work practices and psychosocial case management
• Understanding of community and hospital resources
• Strong analytical and problem-solving skills
• Ability to communicate complex information clearly
• Strong interpersonal and counseling skills

Education:

• Master's Degree in Social Work (MSW) from an accredited institution
